Skilled Nursing Services

Registered Nurses (RNs) provide direct care or supervise care provided by a Licensed Practical Nurse (LPN) or Certified Nurse Aide/Home Health Aide (CNA/ HHA).  They manage, observe, and evaluate patient care and teach the patient and his/her family caregiver how to maintain their care at home. Examples of skilled nursing include giving IV drugs, shots, or tube feedings; changing wound dressings; and teaching about diabetes care.
All patients have a care team that is initiated by a Registered Nurse Case Manager (RNCM). The RNCM will coordinate with all other disciplines and your physician in the development of your specific plan of care.
